Ethiopian Airlines Flight 409 (Beirut to Addis Ababa) crashed into the Mediterranean Sea after takeoff from Rafic Hariri International Airport on 25 January 2010, killing all 90 aboard. Lebanon’s Civil Aviation Authority’s final report was hotly contested. The report blamed the flight crew’s mismanagement of the aircraft’s speed, altitude, headings and attitude through inconsistent flight control inputs resulting in a loss of control and their failure to abide by CRM principles of mutual support and calling deviations hindered any timely intervention and correction.
All ninety remains were recovered from the Sea and identified.
